Wellbeing for Life Facilitator support the Wellbeing Manager and Area Lead with the overall development and implementation of the service. This will include management of Wellbeing Volunteers and Community Health Champions and to broker and nurture local relationships and pathways with partners and organisations from all sectors.
The post holder will promote and support a light touch caseload of clients to access lifestyle and behaviour change support to promote physical and mental wellbeing. The post holder will represent the service at local partnership/ community groups and work closely with the Wellbeing Practitioners to implement key actions and feedback. The post holder will support and undertake asset mapping with the Wellbeing Practitioners to ensure joined up approach service development and delivery.
Job Purpose
To apply asset based community development, engagement and consultation techniques as appropriate to engage the target groups/individuals.
To support the development of new activities for the community based on identified need.
To promote and deliver health improvement programmes and training to individuals and groups to support the delivery aims of the programme.
To ensure appropriate information and resources are available for members of the public and staff in order for individuals to make the correct choices to aid them with changes to their behaviour that have a positive impact on their health.
To represent the service and partners at community group meetings and work closely with the workers to implement key actions and feedback.
Job Description
- To provide signposting support and navigation for clients to access a range of local services, ensuring they access the right support, at the right time, in the right place.
- To source a holistic package of support to assist the development of skills and knowledge and in particular assisting individuals to progress and achieve their health goals.
- To apply solution focused problem solving help across the wider determinants of health.
- To signpost or refer individual clients to a range of health improvement lifestyle intervention, primary and secondary care services and voluntary and community support.
- To apply asset based community development techniques and undertake research to support access to appropriate services.
- To work with partners, including DCC Culture and Sport and local Primary Care Services, and existing groups to develop the referral pathways for support.
- To support the development of new services/activities/groups for the community based on identified health need.
- To promote activities, including managing events and publicity campaigns.
- To promote and administer micro grant programme assisting with form completion and setting up of local grant panels.
- To line manage Volunteers and Community Health Champions and develop the delivery model for the service based on identified competencies required to deliver the client journey.
- To manage/broker/nurture a range of local relationships with partners of all sectors.
- To carry out service evaluation in line with the project plan and learning from best practice.
- To prepare suitable resources for the successful delivery of activities.
- To be responsible for data management of clients including inputting information onto databases and creating reports.
- To work flexibly as part of the team and to provide cover for colleagues as required maintaining appropriate staffing levels across the wider programme.
- To ensure the service operates effectively and efficiently.
- To collate and present verbal and written reports as required.
- To contribute to the development of systems and procedures, and the whole team approach in meeting quality and performance targets.
- To assist with the evaluation of services and sessions as appropriate.
- To conduct personal, venue and activity risk assessments as required.
- To demonstrate innovative and creative practice to engage with vulnerable/disadvantaged groups and utilise support from the volunteers in engaging with communities.
- To audit and maintain resources, stock and equipment levels.
- To contribute to knowledge sharing and learning before, during and after projects, and to the development of a culture that helps colleagues become confident in using the appropriate technology and develop skills.
