Responsible to: Team Manager.
Hours: Part time, 22.5 hours per week, some flexible working will be essential, including some evening working and occasional weekend working.
Contract: Fixed term until 30th November 2020.
Probationary period: This position is subject to a 26-week probationary period, satisfactory references, Mind’s policy on the Protection of Vulnerable Adults and an enhanced CRB check and subsequent re-checks.
Job purposes:
We are seeking a highly motivated individual to provide a person-centred service for adults who support/care for an adult (family member or friend) who is experiencing mental ill-health. You will be part of a new project which offers two strands of service delivery the other being support to parents/carers where the child is not accessing mainstream mental health services.
A proportion of the role will be to work in partnership with TEWV NHS Foundation Trust Crisis Services to identify hidden carers and those most in need. Being present when families present in crisis at the Crisis Assessment Suite, to be able to undertake visits with the Crisis Team to provide immediate and follow up advice and support to families.
Providing mental health and emotional wellbeing support to carers. Supporting carers to feel more valued and supported whilst recognising their role in the wider community. Creating opportunities for carers to participate in training, education, volunteering and employment.
Main duties and Responsibilities:
1. To complete assessments with individuals working collaboratively to identify strengths and challenges in their caring role.
2. Developing a goal based plan with the carer and/or family, tailoring the support needs to the individual care/family.
3. To actively engage in partnership working with the Crisis Team and Crisis Assessment Suite at Roseberry Park.
4. To provide emotional support, psychological coping and self-care techniques for carers.
5. To actively promote opportunities for carers to enable them to have a life of their own, such as training and work opportunities, personal budgets, respite breaks etc.
6. To provide advice and information around mental health conditions, access to mental health services and available interventions.
7. To provide advice and guidance on money or benefit issues and refer on for specialist advice where needed.
8. To provide information about the right to access a statutory Carers’ Assessment and support carers to access them
9. To increase access to the service through promotion with GP’s, health and social care agencies and carers events such as Carers Week.
10. To promote awareness, knowledge and understanding of the needs of carers/families to GPs, mental health professionals etc in order to ensure they are understood, valued and able to access help.
11. To represent the service and Middlesbrough and Stockton Mind at external meetings and events as agreed.
12. Record information in line with the organisations policies and procedures.
13. To effectively manage and respond to risk in line with the organisations Clinical Risk Assessment and Risk Management Procedure.
14. Support carers to complete appropriate tools to measure and assess wellbeing.
15. To work within the policies and procedures of Middlesbrough and Stockton Mind at all times.
16. To take part in support and supervision as required.
17. To take part in training and development activities as required.
18. To undertake other tasks commensurate with the position as required.
