Job Purpose
Wellbeing Practitioners will work with individuals and vulnerable groups aged 16 years and above to encourage participation and manage a caseload of clients to support them through behaviour and lifestyle changes and to assist them to achieve their goals.
The Wellbeing Practitioner will work in a range of settings in geographical patch/location to support the development and delivery of focused outreach work and signpost/referral pathways.
Job Description
1. To work with groups and individuals aged 16 years and over from the community who need support to make behaviour and lifestyle changes.
2. To apply solution focussed problem solving to deal with non-specific queries in relation to lifestyle issues including healthy eating, mental wellbeing, physical activity and smoking.
3. To manage a caseload of individuals and maintain regular supportive contact with each person to assist them to achieve their goals.
4. To encourage and facilitate the development and delivery of personal health action/activity plans.
5. To source a holistic package of support to assist the development of each individual.
6. To assist with service marketing and promotion activities, including supporting events, cancer, mental wellbeing and publicity campaigns.
7. To asset map the local areas and signpost clients to relevant services.
8. To generate a working caseload of clients in groups or as individuals by applying asset based community development, engagement and consultation techniques as appropriate.
9. To deliver mini health checks to engage with individuals in the community and support community events.
10. To work as part of a team to develop a programme of health related activities for the local community.
11. To deliver a range of interactive health workshops.
12. To assist with the evaluation of services and sessions as appropriate and learn from best practice.
13. To work with relevant partners and professionals to support the service development and delivery.
14. To undertake risk assessments on home visits/venues/activities as required.
15. To work in collaboration to develop volunteers’ skills and confidence and provide them with any necessary support.
16. To demonstrate innovative and creative practice to engage with vulnerable/disadvantaged groups to enable health and wellbeing improvement.
17. To be responsible for data management of clients including inputting information onto databases and generating reports.
18. To contribute to data collection, the writing and creation of reports, health improvement information material and maintain required performance, evaluation and record management systems, to evidence progress and impact.
19. To contribute to knowledge sharing and learning before, during and after projects, and to the development of a culture that helps colleagues become confident in using the appropriate technology and develop skills.
20. To collate and present verbal and written reports as required.
21. To contribute to the development of systems and procedures, and the whole team approach in meeting quality and performance targets.
22. To contribute to administrative systems and record appropriate data where required.
23. To operate within commissioners guidelines, adhering to training, monitoring and financial requirements.
24. To plan and prepare suitable resources for the successful delivery of activities.
25. To audit and maintain resources, stock and equipment levels.
26. To work flexibly as part of a team and to provide cover for colleagues as required maintaining appropriate staffing levels within projects.
General

8. To undertake any training and development deemed appropriate.
9. To undertake any such duties required by your Senior Manager or PCP Senior Manager.
