Carers Together Foundation are seeking a Carers Information and Support Worker to work on an outreach basis in James Cook University Hospital and other community hospitals providing a range of information and support services to carers.
Working in partnership with professionals across the South Tees Hospitals NHS Foundation Trust, the postholder will identify and make contact with carers, raising awareness and promoting the inclusion of carers in the admission and discharge planning processes.
To succeed in this role, applicants must have knowledge and experience of working on an outreach basis in a health and social care setting, experience of carrying out assessments and support planning with clients and have an understanding of relevant legislation and the issues affecting carers.
