Carers Together are seeking a Young Adult Carer Transitions Co-ordinator to provide a range of information and support services to young adult carers aged 15-25 years to support their transition to adulthood.
Working in partnership with the Young Carer Service provider and a range of voluntary and statutory organisations across health and social care.
To succeed in this role applicants must have knowledge and experience of working in a health or social care setting in addition to knowledge and understanding of safeguarding children and young people and vulnerable adults. Working in a multi-agency setting, having experience of carrying out assessments and support planning and an understanding of relevant legislation and issues affecting carers are also key elements of the job role.
