Wellbeing Facilitator
Salary: Band 5, Salary £21,136
Located: North Durham Hub with significant outreach
37 hours per week
Fixed Term until 31 March 2022

Would you like to help people find the connections they need to improve their lives? Do you have experience managing volunteers and community health campaigns? Can you work effectively with a variety of partners, community and voluntary organisations?

The successful candidate will promote and support a light touch caseload of clients to access lifestyle and behaviour change support to promote physical and mental wellbeing. They will represent the service at local partnership/ community groups and work closely with the Wellbeing Practitioners to implement key actions and feedback. Supporting/undertaking asset mapping with the Wellbeing Practitioners to ensure joined up approach service development and delivery.
