Senior Practitioner: Hospitals

£25,841 per annum
Carers Together Foundation is an organisation dedicated to improving Carers’ quality of life. We listen to Carers, give them a voice, provide information, practical and emotional support and promote Carers’ rights across the South Tees area.
We are seeking a Senior Practitioner to work on an outreach basis in James Cook University Hospital and other community hospitals providing a range of information and support services to Carers.
Working in partnership with professionals across the South Tees Hospitals NHS Foundation Trust, the postholder will identify and make contact with Carers, raising awareness and promoting the inclusion of Carers in the admission and discharge planning processes.
To succeed in this role, applicants must have knowledge and experience of working on an outreach basis in a health and social care setting, experience of carrying out assessments and support planning with clients and have an understanding of relevant legislation and the issues affecting Carers.
This role is based at our offices in Middlesbrough and Redcar and the postholder will be required to travel to and work in other community settings.
