Job Purpose:
- To co-ordinate the delivery of the SSNP project across Stockton on Tees.
- To provide personalised assessment, signposting, information and support to clients in Stockton on Tees.
- To liaise with GP’s, health professionals and other partners to support the delivery of effective referral pathways.
- To effectively prepare and complete performance monitoring and evaluation of outcomes.
- To promote the service to a range of agencies and groups.
Job Description:
1. To assist with the overall development of the service and manage a caseload of clients.
2. To ensure the service operates effectively and efficiently.
3. To ensure information about local services, referral options and sources of support is relevant to the clients and kept up to date.
4. To build on the public’s desire for a healthier future by ensuring that self care support is in place for the people of Stockton-on-Tees to enable them to make healthier choices about diet, physical activity and lifestyle.
5. To guide and support individuals in taking ownership of their health and wellbeing, and recognize key areas for change and set effective lifestyle goals.
6. To refer complex cases to GPs or appropriate services for re-assessment.
7. To provide appropriate feedback to referrers.
8. To ensure diary is managed effectively through prioritisation, organisation and planning.
9. Work with partners and existing groups to develop the referral pathways for support; particularly disadvantaged groups and areas.
10. To manage a caseload of individuals and maintain regular supportive contact with each person.
11. To encourage and facilitate the development and delivery of personal action/activity plans.
12. To follow up with clients to maintain motivation and assess progress against their individualized action/activity plan.
13. To conduct personal, venue and activity risk assessments as required.
14. To be responsible for data management of clients including inputting information onto databases and creating reports.
15. To collate and present verbal and written reports when required.
16. To contribute to the development of systems and procedures, and the whole team approach in meeting quality and performance targets.
17. To assist with the evaluation of services and sessions as appropriate.
18. To assist with marketing and promotion activities, including supporting events and publicity campaigns.
19. To work flexibly as part of the Project team and to provide cover for colleagues as required maintaining appropriate staffing levels within projects.
20. To operate within the policies & procedures of PCP.
21. To carry out all responsibilities with due regard to the organisation’s Equality and Diversity Policy.
22. To undertake any training and development deemed appropriate.
23. To monitor & maintain Health & Safety and security within the workplace.
24. To undertake any other such duties which are deemed appropriate by your Line Manager.
